As an integrated team we provide a range of services for children, families and young people across Stoke-on-Trent.

What we offer

In Stoke-on-Trent all families will receive the universal offer, which includes: 

  • Contact from the Health Visiting team at around 28 weeks of pregnancy.
  • A new born visit 10–14 days after the baby is born.
  • A mother and baby review when the baby is 5-8 weeks old.
  • Contact from the Health Visiting team when the baby is 3-4 months old.
  • A 9–12 months old review.
  • A 2 year–2 and a half years review.

Some families may require more support. Their Health Visitor will help identify the level of support which the family requires and if needed, we will talk families through additional offers of support available to them

Our School Nursing team provides a range of services for young people aged 5-19 and their families, such as:

  • Transition from Health Visiting services at 5 years of age. 
  • Parents of Reception aged children will receive a welcome letter with the contact details for our hub and links to our website.
  • Students in year 6 and year 10 will be sent out letters with lots of interesting health links and information on how to contact the team.
  • Our School Nurses offer assemblies to students in Year 7 to introduce the services available to them, for example ChatHealth and drop-ins at their school.
  • All young people in Stoke-on-Trent high schools can access health advice and support at  our confidential drop-ins, please contact our Hub for further information or check out our schedule for drop-ins on the website.
  • We will support pupils and families with medical conditions, care planning and coordinating services.
  • We offer training to education/teaching colleagues across the city on Asthma, Epilepsy and Anaphylaxis
  • The service will support applications for Continuing Care Funding and contribute to Special Educational Needs and Disability (SEND) Assessments
  • Clinics are provided throughout the city for Enuresis (bed wetting) and advice about constipation. Please contact our Hub for advice on these conditions.
  • Health promotion - if you require health promotion support please contact our Hub to arrange.
  • We support our Children in Care by completing health assessments.
  • We work with other professionals to support the work of safeguarding children and families through Early Help intervention, Child in Need and Child Protection.

ChatHealth

We offer our ChatHealth text messaging service to parents, carers and young people aged 11-19 years.

This service is managed by Public Health Nurses who aim to respond to all messages within 24 hours, Monday to Friday (excluding weekends and bank holidays).
