It includes consultant psychiatrists, psychologists, advanced nurse practitioners, community psychiatric nurses, occupational therapists, support workers, peer support workers and social workers. NHS England Case Managers works in partnership with the team, and more integrated with providers, service users and carers and provide robust Access to Care assessments to ensure alternatives to admission are appropriately and adequately considered to prevent avoidable admissions, working alongside secondary care providers and criminal justice agencies.
