The Community Stroke Rehabilitation Team provides support to patients with a new stroke who are aged 18 or older, are medically stable and have rehabilitation goals requiring specialist input on discharge from hospital, and are registered with a GP surgery within East Staffordshire:

We provide specialist treatment, support, information, and advice to people following a new stroke, and the majority of therapy will take place in your own home.

We ensure patients receive a high quality of care by working with you to achieve your own personal goals, listening and respecting your views, and helping to improve your quality of life.

We encourage self-management programmes and strategies to promote independence for life post-stroke.

Support is offered in important areas of lifestyle, such as driving, work, and social life.

We will help in identifying and addressing specific goals to improve your independence and quality of life, and provide advice for you, your carer, or family and friends.

Information and support is provided to help you access community and voluntary services, and we are a point of contact for support and advice for you and your family.
