The team deliver care for people who have been identified by Integrated Care Boards as having a “primary health need”, which is assessed by a panel of health professionals. Such a substantial, ongoing care need is classed as a long-term condition, the care for which will be fully funded by the NHS. Unlike social and community care from the local authority, there are no costs associated to the service user.
The team acts as an interface between several health and social care pathways.
