We are excited to announce a new patient portal in partnership with Patients Know Best (PKB) to help you access your health records and get more involved in your recovery journey.
PKB is a secure online portal that allows you to store and access your health records. You can also use PKB to share information, such as your care plan, with other healthcare teams, carers, family, and friends, and to track your progress over time.
This free service is accessible anytime, anywhere on a mobile phone, tablet, or computer, meaning your information is at your fingertips, whenever you need it 24/7.
We believe that PKB can help you in the following ways:
Empower you to take control of your health. PKB gives you easy access to your medical information, so you can make informed decisions about your care.
Improve communication with your other healthcare teams. PKB allows you to share your information with other healthcare professionals easily. This can help them to better understand your needs and provide you with the best possible care.
Support your recovery journey. PKB allows you to track your progress over time and see how you are doing. This can help you to stay motivated and on track with your recovery goals.
If you are already registered to PKB, you will begin to see your patient records from MPFT from Monday 20 November 2023.
How to use PKB

Frequently Asked Questions
You will be sent an email to the email address you provided when you registered.
Questionnaires: These tools help you and your healthcare team track your recovery journey. We will use the information you provide to make changes to your care, if needed.
Allergies: A list of all the things you are allergic to.
Appointments: A list of all your upcoming appointments with our mental health services. You can see appointments for up to 12 months from the date you register.
Documents: Letters from your doctor, other healthcare professionals, and your care team. You can see all of these letters from the date you register. Older letters may not be available, unless a healthcare professional has chosen to share them with you.
Care plans: A plan for your care that is created with you and your healthcare team. You can see your care plan as soon as you register. We encourage you to review your care plan to make sure that it is what you and your healthcare team agree on.
Please note that not all services will be using all of the above options.

This will help you to learn more about your lifestyle behaviours and whether you would benefit from making any changes. Yes. The PKB software meets NHS requirements for health record systems and is fully compliant with the General Data Protection Regulation (GDPR). Only the minimum level of data necessary to verify you has been uploaded. All personal and health information stored is encrypted.
Patients Know Best is an optional service and requires you to "opt in". It provides choice to patients who wish to interact with the hospital in a digital format; it will not affect the care that you receive from the Trust. If you do not wish to use the portal, you can ignore the invitation and you will continue to receive your hospital correspondence in the usual way.
If you want the family member or carer to have access to your record they can be added in the ‘sharing’ section. This will allow them to set up their own account and allow you to control the level of access they have to your record. For more information visit the PKB manual.
Some patients may not have any information in their records when they first register, however this will change over time as you attend appointments. You can still add your own information, such as tracking your symptoms or recording any medication you take.
Currently this service is only available to MPFT patients who have been active referral to Mental Health, Eating Disorders, Perinatal Mental Health, District Nursing and Musculoskeletal (MSK) services. As the service area widens, we will inform eligible patients via email and letter.

Get help registering for PKB and learn how to navigate the platform. These informal drop-in sessions are hosted by our partner provider Pulse 4 Music. New dates and venues coming soon.
