MPFT’s Staying Well Service offers care and support to people aged 55 and over in the south of Staffordshire with mild to moderate levels of frailty, who are at risk of falls, are socially isolated or taking multiple medications.
Service users may be invited to join one of the service's regular Hubs, held across southern Staffordshire in easily accessible locations within local communities. They provide a ‘one-stop shop’ approach to enable care and support to be easily available. This video follows the team at one of these Hubs (held in Codsall) and spends time with a service user about why the Hubs can help to make a difference.
